Apple has announced the Apple Ultracompact USB Power Adapter Exchange program for iPhone 3G sold in select countries. Apparently, the adapter's metal prongs can break off and remain in a power outlet, creating a risk of electric shock. There were even some reports of detached blades involving a very small percentage of the adapters sold.
So, Apple has voluntarily decided to exchange the ultra-compact power adapter for a new (and redesigned adapter) for no extra charge.
Only the adapters bundled with iPhone 3Gs in countries like US, Japan, Canada, Mexico and several Latin American countries have this issue. Replacement adapters have a single green dot on the bottom to distinguish them for the older ones.

The exchange process starts Friday, October 10, 2008.
